Do I need a separate PTAN number (CMS supplier number) and NPI number for my affiliate locations?Yes. According to the NSC, “The supplier must enroll separate physical locations it uses to furnish Medicare-covered DMEPOS, with the exception of locations that is uses solely as warehouses or repair facilities.”
